Service Learning Proposal Form

Directions for SLP Form Submission: 

1. Ensure that you secure a sponsoring teacher for your activity. 

2. Fill out the SLP form completely and with as much detail as possible. Click the form on the right to expand it. Use your GDOE-assigned email address to access the form.  

3. You should receive a response within 3 business days regarding the status of your proposal. 

4. You may email SSHS@GDOE.NET to follow up on the status of your proposal or to submit relevant documents.
